This module aims to enhance the healthcare professional’s knowledge and understanding of long-term health conditions and their management to enable service users to live with the conditions and improve their health and wellbeing.
As a stand-alone module, it can be used to inform practise regarding the management of patients with long term conditions and ways in which empowerment can be used to influence health behaviours to improve patient outcomes.
Previous students have also studied this course as a module which has introduced them back into higher education.

- Strategies to manage long term health conditions e.g - expert patient programmes, integrated care.
- Bio-psychosocial and environmental factors which influence the conditions and affect health and well-being.
- Appropriate theories including organisational management, whole systems, change management, partnership and collaboration.
- Ethics in care, quality evaluation and outcomes measurement.
- National and international evidence and policy that influences practice.
- The evidence base for care and enhancing health and well-being across the lifespan.
Students should be aware they may encounter ethical issues in learning and teaching while undertaking this module and should refer to either the Award/Module Leader for further information.
- Lecturers and seminars
- We invite specialist practitioners and University lecturers to deliver sessions.
- Via face to face classroom or virtually via our online blackboard system. There will be a mixture of live and recorded sessions.
- You will be expected to take part in-class discussion and to share your experiences.
This module requires you to study for 36 hours at University during lectures or seminars and spend up to 64-hours undertaking self-directed study. You are also required to be able to evidence 100 hours of clinical practice
You must be a health professional employed in an area where you can meet the clinical practice element of 100 hours.
Formative Assessment: you will complete a case study on which to base your assignment by week 4 of the module.
Summative Assessment: you will complete a 3000-word assignment that identifies an individual with at least one Long term Health Condition and then critically analyses and evaluates how the professional works with the service user and other professionals to enable them to improve their health and well-being.
Practice Assessment: you will complete 1 clinical learning outcome verified by a practice assessor/supervisor of achievement of one clinical learning outcome, documented within the Clinical Outcomes Achievement Record-this will be based around a teaching session for patients or team members.
You will be assigned an academic supervisor who will assist you with your academic work.
